Who we are: Operation CARE was designed as a gap service to meet the acute needs of those who are facing layered challenges related to substance use disorder through connecting them with immediate peer support services.
CARE Coaches respond to referrals in a variety of medical settings, including emergency departments, outpatient clinics, and labor and delivery units.
The ability to quickly respond to an individual in their most desperate hour allows for the individual to receive support and attention in such a way that medical personnel is often not equipped to, thus offering much-needed support and relief to the personnel as well.
What we offer:
The malleable structure of Operation CARE allows our team to respond to the specific needs of varying communities, hospital systems, and patient populations. We offer two service lines under the CARE umbrella: inpatient referrals, known as bedside (moderate-high acuity), requiring same day or next day dispatch, and outpatient referrals, known as clinic (low acuity), which do not require the same level of immediacy. The average length of the Operation CARE program is 90 days at which point the participant can choose to continue longer term peer support services through HardBeauty or any other agency. Operation CARE was not designed to be a funnel to HardBeauty services.
The process: Providers may refer willing individuals to CARE services through utilizing a secure and HIPAA compliant online referral form. Referrals are fielded 7 days a week from 8a-8p.
Our intake team will respond to referrals within an hour of notification. For clinic referrals, providers can expect a confirmation email, whereas for bedside referrals, providers can anticipate a call from intake to collect any contextual information needed to help determine the scope and celerity of need.
